3 ERRORS kept on recurring. Array to string conversion, Undefined offset: 1, etc

this error on the log file keeps on recurring in the FORUM error log,.. every time a member makes an action.. that means ANY action made.. posting a new message, replying a PM.. modifying a message.. AS IN.. any ACTION.. these 3 ERRORS kepts on reappearing.

i already did fix suggestions like:

but i still get the same ERROR
