I'm trying to allow users to make a registration request, by filling out the registration form, but not having the user be activated until approved by the admin.  From every possible combination of settings this does not seem possible from my efforts.

I've set the SMF setting to Approval, but this seems to have no effect.  I've disabled the Mambo user self registration but then the registration form is unavailable.

Has anyone successfully done this with SMF/Mambo short of creating their own registration form and hacking it into the SMF Integration module?
